Integrated Care Across the Lifespan
This program will examine and promote evidence-based practices for the integration of behavioral health care service competencies into primary care settings across the lifespan. Model national and local programs fostering collaborative care by primary care and behavioral health providers will be described. Strategies to optimize behavioral health care connections and capacities within the primary health care setting will be explored. The program will discuss a number of innovative San Diego based efforts that integrate behavioral health. Special focus will be paid to clinical rationale and outcomes of behavioral integrated model of care.

Activity Name: Integrated Care Across the Lifespan
Description: This program will examine and promote evidence-based practices for the integration of behavioral health care service competencies into primary care settings across the lifespan. Course Objectives: At the conclusion of this activity, participants should be able to:

  • Describe the Center for Medical & Medicare Services? Transforming Clinical Practice Initiative (TCPI) model for inter-professional collaboration.
  • Describe techniques for identifying, supporting and engaging patients in appropriate behavioral health services linked to their primary care medical home.
  • Define strategies to encourage organizational engagement and reforms to incentivize integrated care delivery.
  • Apply knowledge, skills and attitudes to better collaborate with colleagues and patients in efforts to achieve better outcomes for patients of all ages.
  • Describe the clinical value in integrated care for better patient outcomes in the primary care setting.
