Needs Assessment:
Gaps include: 1. Health care providers are uniquely positioned to discuss community resource options with patients and their families, however many are not well informed on the many resources available.
2. Misidentified behavioral health issues can negatively affect mortality rates among the Baby boomer generation. Late-life depression increases risk for medical illness and cognitive decline. Unrecognized and untreated depression has fatal consequences in terms of both suicide and nonsuicide mortality. However, many physicians are not trained on early intervention methods and are lacking in knowledge on how initiate behavioral health discussions.
3. Alzheimer's disease is a growing health problem among older adults, with an estimated 14 million affected patients by 2050. The development and refinement of disease management techniques and care giver education will be crucial to meeting the needs of this growing population.
