This evaluation/credit form is for attendees that participated in the TCOYD Conference and Health Fair that would like to claim credit for the overlapping CME/CE sessions.
CME/CE registrants in the TCOYD Making the Connection CME program should claim credit through the TCOYD: Making the Connection San Diego 2018 Evaluation listing.
Only course attendees should complete this evaluation and claim credit for this activity.
You will be asked to supply the
access code
you received at the conference to submit this evaluation and receive your certificate. The access code can be found in the handout materials.
After completing the evaluation(s) and attesting credits claimed, a PDF certificate will be available for immediate download.
This certificate will also be stored in your CME California transcript, accessible anytime by logging in to CMECalifornia.com and selecting the "Transcript" option from the left menu bar.
